ShyamsundarR 90d05603c9 Updated guide with 3.13 upgrade procedure

Although the procedure remains the same, recreating the text with
updated version/release numbers makes for an easier reading IMO.

Hence retained the text as is (like in 3.10 -> 3/12) but changed
the version numbers in the text appropriately.
