Source is from the glusterfs release-5 branch.
Also modified upgrade guide to reflect options that have
been removed from the code and are reflected in the release
notes as well.

Attempted to not copy the guide as is, like previous iterations
and instead pointed to the 4.1 guide, as procedure remains the
same.

Upgrade guide did not specify stopping and restarting glustereventsd
and these instructions are now added to the 3.12 and 4.1 upgrade
guides, as these are the current supported releases.
